Position: PT Service Coordinator
Reports to: Program Director
Hours: Part time
Duties and Responsibilities:
- Assess, plan, implement, coordinate, monitor, and evaluate services required to meet a participants needs.
- Inform, and refer participants to available community services.
- Participate in the completion of comprehensive assessment and plans of care.
- Writing service coordination notes in the participants health record upon service commencement, when significant changes occur, and at least once per month.
- Provide, as appropriate, coordination of medically related physical, mental, and behavioral service needs at time of a participants enrollment, during care in the program, or at time of discharge.
- Assist with any personal, social, family, or adjustment problems participant may experience at the program.
- Renew all program related forms such as CDBG, Kit Clark, DOE nutrition, and others annually.
- Assist and provide back up support for direct services as needed in the Department.
- Initial entries and update database on ETO software.
- Perform other related duties as assigned by director
- The person has to be a Licensed Social Worker. College degree or previous experience in case management, social services, working with children, or adults in a professional capacity, or related areas preferred.
- Bilingual (Spanish and English, bicultural or have knowledge about the cultural values and special needs of the Latin American and Caribbean population living in the greater Boston area).
- Great interpersonal skills with emphasis on both verbal and written abilities to communicate effectively.
- Ability to work within a team approach environment and self-motivated to work independently.
- Flexibility and willingness to assist and work with staff and volunteers in other areas in order to ensure quality of service and accomplish overall goals/objectives.
- Ability to keep information confidential.
- Computer knowledge in Microsoft word, Power Point, Excel, Publisher.
- Organized, detail oriented and ability to handle multiple tasks.
- Ability to effectively plan, coordinate and implement work.
- Ability to facilitate, monitor and documents group events, client feedback as well as group interaction.
- Willingness to learn and participate in improving quality of services and efficiency by attending safety trainings, seminars and other staff development opportunities as needed.
Position: Therapeutic Activity Director
Reports to: Program Director
Hours: Monday through Friday, 20 hours (PT)
Dept.: Elder Department.
Duties and Responsibilities:
Develop and implement therapeutic activities for each participant based on their individualized care plan.
· Supervise, recruit, and provide orientation to volunteers and inters that assist with therapeutic activities, in addition, maintain volunteer records.
· Plan, facilitate, schedule, and create a monthly calendar of all activities and events, and post on all boards visible to all staff and participants.
· Contribute to each participant’s comprehensive assessment and care plan before 15 days of their enrollment.
· As a team, revise individual care plans for participants a monthly or as needed basis, and make the changes necessary to meet their needs.
· Keep all records up to date on an as need basis.
· Write therapeutic activity progress note at least one time per month or more often if necessary in each participant’s health record. Document their participation in recreation activities, progress, and/or any changes.
· Coordinate, and supervise transportation to provide the highest level of service. Maintain a current list of clients for transportation purpose and attendance.
· Track participant’s daily attendance with detailed information such as absences, medical appointments, vacation and report it to Program Director, as well as inform Nurses and Program Aides.
· Provide participants with any assistance needed in activities of daily living and supervises other program staff in the provision of such assistance.
· Maintain constant communication with Program Aides, RN, LPN and other staff with progress, issues and need of participants in order to assure continuity of care.
· Create new partnership in the community to build relationships and bring a variety of activities to program.
· Follow up with protocols, policies and emergency procedures in the Elder Department as a needed.
· Weekly attendance to staff program planning meetings and annual trainings as required by regulations.
· Coordinate and perform outreach activities previously discussed with Program Director, to promote services, and recruit Participants for the department.
· Assist participants by providing translation/interpretation.
· Collect Data for ETO software as a needed.
· Perform other related duties as assigned by program Director
· Therapeutic recreation specialist or activities professional certified by an accrediting body recognized by the Department; or qualified occupational Therapist; or occupational therapy assistant ; or
· Has at least two years of experience working in a social or recreational program within the last five years, one of which was full-time in an activities program in a health care setting.
· Completed or renewed training in CPR and basic first aid within the last year.
· Bilingual (Spanish/English) and bicultural or familiarity with the cultural values and special needs of the Latin American and Caribbean population living in the greater Boston area.
